Electronic Notification Form and Privacy Impact Assessment (DPIA)

Health research projects planned to be conducted at OUS must be reported to the Data Protection Officer and Chief Information Security Officer in an electronic notification form. The form must also be used for significant changes that require ethics committee (REK) approval.

The following study documentation must be uploaded in the form:

Copy of REK application with all attachments
Protocol/project description
Information and consent form
REK approval, if the project has already been approved

As part of the completion of the electronic notification form, researchers will be asked to fill out a self-declaration on DPIA, to clarify whether a separate data privacy impact assessment (DPIA) is required for the use of personal data.

For projects requiring a privacy impact assessment, the DPIA template must be completed and uploaded in the form together with the documentation mentioned. For assistance, send an email to godkjenning@ous-hf.no.
